When considering a travel career, you need to choose a company that listens and understands your personal and professional needs. Plus, you want an experienced company that provides outstanding customer service.

1) The first step in the process is for you to Apply NOW!  We need your contact information.  It only takes about 15 seconds to fill out the information we need.

 

2) For those with an updated resume you will want to email your resume to us HERE! Once we have your resume, we can begin the process of getting you into our database to begin to look for jobs that meet you qualifications.

4) References (3-5 professional references from supervisors or managers)

 

5) Once you have accepted an assignment with Summit Medical Staffing, the following items will be required in order to compete our placement process:

 

  • Copy of a Valid Photo ID (Drivers License or Passport)

  • Copy of your Social Security Card

  • Copy of all Required Certifications (Front & Back) (BLS, PALS, ACLS, ECT)

  • Copy of the Appropriate State Licenses (Front & Back)

  • Copy of Immunization Records (Must Include MMR,TB, HEP B, Varicella)

  • A completed ACH Direct Deposit Form including a copy of a blank check

  • I-9 Employment Eligibility Verification

  • W-4

  • State Worked-In Tax Form

  • Medical Records and Background Release Form

  • General Health Statement (Must signed by a Physician, NP, or PA within the last year)

  • Guidelines Compliance Form

  • Age Specific/Emergency Contacts Form

  • Insurance Paperwork (Must return signed applications whether accepting or declining enrollment)

  • Competency Testing (Sent through email)

  • Drug Test (Chain of Custody or Authorization Form)
