Once you have accepted an assignment with Summit Medical Staffing, the following items will be needed to get you started:

  • Copy of a Valid Photo ID
    (Drivers License or Passport)

  • Copy of your Social Security Card

  • Copy of all Required Certifications (Front & Back) (BLS, PALS, ACLS, ECT)

  • Copy of the Appropriate State Licenses (Front & Back)

  • Copy of Immunization Records (Must Include MMR,TB, HEP B, Varicella)
